The aim of Person Centred Care is to ensure that the client is an equal partner with our care staff in assessing, identifying options for and delivering the most appropriate package of care for that individual across organisational boundaries.

From the initial contact with our administration team it is vital to us that our clients and their families are as actively involved as they choose to be in designing and implementing their care plan that is individual to their needs. Our in depth care assessment will ensure that the cultural, medical and social needs of the client are central to the clients individual service.

Our mission and values are in tune with the personalisation of care agenda, promoting a strong person-centred approach, personal choices and personal outcomes for all our service users. Those whom we support are not simply service users but co-producers of their services, actively involved in service planning, delivery, review and improvement.
