The aim of Person Centred Care is to ensure that our clients are an equal partner in assessing, identifying options for and delivering the most appropriate package of care for them. It involves the provision of full information on all aspects of the client’s needs and available services and requires the client to be treated with respect, courtesy and dignity at all times.

So that we can deliver the most appropriate package of care for each individual we carry out a detailed assessment prior to admission that takes into account the “whole person”.  We will discuss social needs as well as medical needs so that we can provide the total care of the client.  Where possible we will include family and relatives and other health care professionals.
